A suitable donor is not selected by blood group or clinical parameters alone. For many transplants, the recipient’s immune system must also be considered.

Human Leukocyte Antigen (HLA) molecules help the immune system distinguish self from non-self. HLA compatibility can influence the risk of rejection and the interpretation of recipient antibodies.
Accurate HLA typing provides the foundation for donor–recipient immunological assessment and helps transplant teams interpret antibody results in context.
When a biological relationship needs to be established, HLA-based kinship analysis can support pedigree or relationship assessment.
This can be relevant in living-donor evaluation and in situations where biological relationship documentation is required. Results should be interpreted alongside clinical records and applicable legal or regulatory requirements.
India’s transplant law — the Transplantation of Human Organs and Tissues Act (THOTA), 2014 — restricts living donation mainly to “near relatives”: parents, children, siblings, spouses, and grandparents/grandchildren. This rule exists for a good reason: to prevent commercial organ trafficking disguised as family donation.
When documentary evidence (birth certificates, Aadhaar, family photographs, etc.) isn’t enough to conclusively establish the relationship, the law requires DNA-based verification. Depending on the claimed relationship, this can involve autosomal STR profiling (for parent-child or sibling pairs), mitochondrial DNA analysis (for maternal-line relationships), or Y-STR analysis (for paternal-line, male relationships). These methods can support or refute a claimed relationship with a very high degree of confidence — protecting genuine donor families from delay, and protecting the system from abuse.
Form 5 is the official certificate under the THOTA Rules in which a laboratory formally reports the outcome of this DNA/HLA-based relationship testing to the hospital’s Authorization Committee. Without a valid Form 5 report an Authorization Committee cannot approve a “near relative” living donor transplant if documentary evidence alone was inconclusive. In other words, Form 5 is often the single piece of paper standing between a family and a life-saving surgery date.

The Single Antigen Bead (SAB) assay is a Luminex-based method used to detect and characterize antibodies directed against individual HLA antigens.
SAB results can help define the recipient’s antibody profile, identify unacceptable antigens, and support a more informed donor evaluation.
Donor-specific antibodies (DSA) are recipient antibodies that recognize HLA antigens expressed by a particular donor. Luminex-based testing can help identify and characterize these antibodies.
DSA assessment can add an important layer to donor selection by helping the transplant team understand potential antibody-mediated immunological risk. Interpretation should consider clinical context, assay characteristics, antibody strength, complement-binding information when available, and laboratory thresholds.
Legal clearance and a good HLA match aren’t the whole picture — the recipient’s immune system may already carry antibodies against the donor’s specific HLA markers, raised by a prior pregnancy, transfusion, or earlier transplant. This is where Single Antigen Bead (SAB) assay and Luminex-based Donor-Specific Antibody (DSA) testing come in. By screening the recipient’s serum against a panel of individual HLA antigens, this testing detects pre-formed, donor-specific antibodies that could trigger antibody-mediated rejection — often before a conventional crossmatch would reveal a problem.
For transplant teams, this means donor selection isn’t just a legal and genetic question but an immunological risk assessment. Our lab offers Luminex-based SAB/DSA testing alongside HLA typing, so clinicians get a fuller risk picture before finalising a donor.
